چکیده
Objective To compare intramuscular oxytocin, Syntometrine® and carbetocin for prevention of postpartum haemorrhage after vaginal birth. Design Randomised double-blinded clinical trial. Setting Six hospitals in England. Population A total 5929 normotensive women having a singleton Methods Randomisation when birth was imminent. Main outcome measures Primary: use additional uterotonic agents. Secondary: weighed blood loss, transfusion, manual removal placenta, adverse effects, quality life. Results Participants receiving uterotonics: 368 (19.5%) 298 (15.6%) Syntometrine 364 (19.1%) carbetocin. When pairwise comparisons were made: significantly more likely to receive uterotonics than those (odds ratio [OR] 1.28, 95% CI 1.08–1.51, P = 0.004); the difference between oxytocin non-significant (P 0.78); less (OR 0.75, 0.65–0.91, 0.002). Non-inferiority not shown. Use reduced non-drug PPH treatments compared with 0.64, 0.42–0.97) but 0.64). Rates transfusion different. associated an increase maternal effects ability mother bond her baby. Conclusions Carbetocin is different uterotonics. need oxytocin. متن کاملAdverse Effects of Carbetocin versus Oxytocin in the Prevention of Postpartum Haemorrhage after Caesarean Section: A Randomized Controlled Trial
Purpose To compare the incidence of nausea, vomiting, and arterial hypotension between carbetocin and oxytocin to prevent haemorrhage after caesarean section (CS). Methods A randomized controlled trial in term pregnant women undergoing planned CS. Groups were randomized to carbetocin or oxytocin. Blood pressure (BP), heart rate, presence of nausea/vomitus, and need for vasopressors were evalu...
متن کاملA randomised trial of carbetocin versus syntometrine in the management of the third stage of labour
OBJECTIVE Syntometrine is an effective uterotonic agent used in preventing primary postpartum haemorrhage but has adverse effects including nausea, vomiting, hypertension and coronary artery spasm. Carbetocin is a newly developed long-acting oxytocin analogue that might be used as an uterotonic agent. We compare the efficacy and safety of intramuscular (IM) carbetocin with IM syntometrine in pr...
متن کاملPrevention of postpartum haemorrhage with the oxytocin analogue carbetocin.
Postpartum haemorrhage is the leading cause of maternal mortality worldwide: 67-80% of cases are caused by uterine atony. Preventive measures include prophylactic drug use to aid uterine contraction after delivery, thus avoiding severe blood loss and reducing maternal morbidity and mortality. Carbetocin is a synthetic analogue of oxytocin with a half-life approximately 4-10 times longer than th...
متن کاملIntramuscular oxytocin versus intravenous oxytocin to prevent postpartum haemorrhage at vaginal delivery (LabOR trial): study protocol for a randomised controlled trial
BACKGROUND Primary postpartum haemorrhage (PPH) is one of the leading causes of maternal morbidity and mortality worldwide. The most common cause of primary PPH is uterine atony. Atonic PPH rates are increasing in developed countries despite routine active management of the third stage of labour. In less-developed countries, primary PPH remains the leading cause of maternal death. Although the ...
